Deregister as a patient

If you have a new general practitioner, we kindly ask you to inform us by sending an email to uitschrijven@mcloudon.nl. Your new GP will request your medical file from us, and we will transfer it in full.

Patients registered with us who live outside our postal code area will soon receive a letter from us. General practitioner care is under serious pressure; the demand for care is increasing, and the number of older patients is rising. Therefore, it is crucial that we, as general practitioners, ensure that care remains neighborhood-focused. This allows each GP to provide care for patients around their practice and respond promptly in emergencies.

From our patient database, it appears that we have many patients who live (far) outside our district due to relocation or other circumstances. Because patients from outside our region are registered with us, we do not have enough space to accommodate new patients from our own neighborhood.

Medical Center Loudon must make choices regarding its patient database in order to maintain and improve neighborhood care. Patients who live too far outside our service area will be asked to find another general practitioner in their own neighborhood. We understand that this may be uncomfortable for this group of patients. At the same time, we hope for understanding of the challenges facing healthcare and the sometimes difficult choices that arise from it.
